Note: Due to construction in the area, Ben Greiner park is not accessible from Eubank.

2012 Season is underway

The following items are REQUIRED for Athlete Eligibility:

1. APS Physical
Before an athlete is allowed to practice or compete, a completed physical must be on file with the Athletic Department / Trainer. Forms can be downloaded below, or on the APS athletics website.
2. Eligibility Form
Form will be provided by coaching staff, and returned to coach when completed. All information must be filled in and document signed.
3. Concussion Protocol
Parents must attend the Fall Sports parent meeting where information regarding Concussion management will be discussed. A form verifying completion will be submitted to the coaching staff.
4. Athlete Code of Conduct
Available below in pdf format, parents and athletes should read and understand the expectations of an athlete representing Manzano High School. Print out the last page, sign, and turn in to coaching staff.
5. Life of an Athlete
NMAA sponsored online program aimed at increasing awareness of the damaging effects of alcohol on teenagers, and on athletic performance. Athletes must complete online module, and print out completion certificate. Certificate is turned in to coaching staff and must be on file with the Athletic Department.

Manzano High School Athletic Department

It is the responsibility of parents and students to read and familiarize themselves with the terms and requirements of the Albuquerque Public Schools (APS) Athletic and Activity Code. A student or his/her parent/guardian may obtain an explanation of any part of the code from the Athletic Director, Activities Director or Principal.

The parent/guardian and student are required to sign the attached form to acknowledge the code has been read, is understood by the student and the parent/guardian, and they agree to abide by it.

APS Physicals

You must have a physical in order to practice and compete,
even for summer practice.

APS sponsored physicals will be available in the fall, but not in time for summer practice.

Every athlete is required to get a new physical each academic year, even if they had one the previous year. Physicals dated after April 1st are acceptable for the entire following school year.
