Please keep the health office informed yearly of any health conditions, medications, or concerns. We are here to support our students! Also, please ensure contact information is up to date in case of emergency.
Medications At School
If your student requires medications or procedures at school, the proper form must be completed and signed by a physician each school year - see health office form links to the left. If deemed appropriate by a physician, students may self-carry/administer medications. Forms are still required for students self-carrying. Students cannot self-carry/administer medications classified as "controlled" (i.e. most ADHD medications).
All medications provided for use in the health office must include the prescription label. Over the counter medications must be in a new, unopened bottle. We cannot accept expired medications.
Parents are required to pick up non-emergency medications at the end of the school year or the medications will be discarded by health office staff. Reminders will be sent home mid-May.

UNM School-Based Health Clinic
The School-Based Health Clinic is a collaboration between Manzano HS, New Mexico Department of Health, and the UNM Health Sciences Center to provide confidential medical and behavioral health services on campus to students. This includes physicals for athletics. They are located in room (portable) S-16. Phone: 253-0012.
A parent consent form is required yearly for students to use the SBHC

Immunizations
All students must have current immunizations in order to enter or remain in school. Please bring most current records when registering for school.
The Department of Health now offers a website to access your child's shot records, check out VaxView NM.
Health Insurance
APS's Medicaid Department is available to help families enroll in Medicaid or refer for other eligible services. Please call 505-855-5261 or 505-855-9820 (for Spanish speakers) for more information.
